Title: Clinical Pharmacy Technician (Hybrid Position)

The Clinical Pharmacy Technician will support the Clinical Pharmacy team workflow and provide direct telephonic patient care to support overall quality of Comprehensive Medication Management services and Pharmacy related quality metrics for our high risk attributed patient population.

Essential Job functions:

  • Interact with all levels of medical staff to discuss relevant patient information as warranted.
  • Conduct chart review and patient interviews to identify patient specific reasons for poor medication adherence and resolve identified problem per protocol.
  • Perform medication reconciliation for patients recently discharged from the hospital or as requested by the Clinical Pharmacist per protocol. Record medication information into the "Meds" in the electronic health record and update existing information as needed to ensure accuracy of the patient home medication list.
  • Prepare and manage patient education materials/handouts.
  • Make outreach to patients identified from payer medication adherence-related reports.
  • Participate in training and orientation of new employees as needed.
  • Comply with the Summit Health Management incident reporting policy and procedures.

Education, Certification, Computer and Training Requirements:

  • Education: High School diploma or equivalent required
  • Experience: Hospital Pharmacy technician experience preferred.
  • Certification/Licensure: Registered Pharmacy Technician required, within 6 months of employment in this position.
  • Preferred: Completion of a certified pharmacy technician training program or a passing score on the Certified Pharmacy Technician Examination offered by the Academy of Pharmacy Technicians or the American Society of Health Systems Pharmacists.
  • Valid Driver’s license required.
