Personal Health Coordinator

The Personal Health Coordinator is responsible for overseeing health coordination services to members of the Seabury At Home program, enabling members to remain in their own homes by establishing individual wellness profiles to assure pro-active intervention when necessary through the means of temporary or long-term rehabilitation services. 2+ years related experience and Masters Degree in Social Work or LCSW preferred. This is a full-time position working 40 hours/wk with a rotating on-call schedule.
