Job Summary

The Case Manager provides support to Oakland County residents, who are on an Assisted Outpatient Treatment (AOT) order through the Oakland County Probate Court. The Case Manager provides support, education, connection to community resources and advocacy to persons served, while adhering to the court ordered services.

Essential Functions

Under general direction and as assigned:

  • Maintains a maximum caseload of 25 individuals, with primary focus being on individuals not served through the OCHN provider network.
  • Assesses person’s needs through appropriate assessment tools and develops / implements treatment plans / recommendations for individuals.
  • Provides intensive supportive interventions one or more times per week with individuals / families on their caseload.
  • Coordinates therapy, medication, and additional needs with hospitals, Probate Court, and other service providers.
  • Be familiar with community resources available to Oakland County residents.
  • Coordinates / Collaborates with Law Enforcement as needed.
  • Maintains timely and accurate records in Electronic Health Record.
  • Provides Crisis Intervention as needed.
  • Files petitions as needed.
  • Attends court hearings as needed.
  • Has knowledge of the Michigan Department of Health and Human Services (MDHHS) programs and eligibility requirements.
  • Navigates Oakland Community Health Network (OCHN) processes, including mental health and substance use screening parameters.
  • Assists individuals without insurance in exploring Medicaid eligibility and application procedures.
  • Assists individuals who are moving out of county, with connection to services.
  • Develops and maintains working relationships with community partners, including the Oakland County Probate Court, crisis providers, hospitals, provider network, etc.
  • Provides Training for Probate Court, provider network, community members and law enforcement.
  • Collaborates with other OCHN teams regarding department cross-over for assistance.
  • Participates in agency meetings, including AOT Workgroup / Steering Committee.
  • Other duties as assigned.

Job Requirements and Qualifications

Education:

Bachelor's degree in Social Work, or Psychology

Training Requirements (licenses, programs, or certificates):

LLBSW / LBSW, preferred

Experience Requirements:

At least two years working in the behavioral health field, focused on adults with serious mental illness. Appropriate bachelor-level internships may be considered.

Job Specific Competencies/Skills:

  • Ability to establish and maintain rapport.
  • Demonstrated ability to work effectively in a team environment, as well as independently.
  • Ability to utilize Motivational Interviewing, as well as trauma-informed practices.
  • Have knowledge of health insurance practices and be able to direct clients in using services available to them through their health plans.

Knowledge Requirements:

  • Knowledge of the justice system, especially Probate Court proceedings.
  • Knowledge of services and support in a crisis continuum of care.
  • Knowledge of the Petition and Certification processes.
  • Knowledge of Recipient Rights.
  • Knowledge of LOCUS, preferred.
  • Knowledge of public mental health service delivery systems in Oakland County.
  • Knowledge of HIPAA.

Additional Information

(Travel required, physical requirements, on-call schedules, etc.):

  • Must have available means of transportation to and from OCHN and for required offsite meetings or site visits.
  • Must be available for meetings and events which may occur outside of standard office hours.
  • Work requirements include meeting individuals in the community, hospital, court, and / or office setting.
  • Hybrid (onsite/remote) work schedule available.
