ICWP CASE MANAGER

JOB SUMMARY:

The ICWP Case manager is responsible for the overall enhanced primary case management of those individuals who have been assigned to their caseload.

Non-Exempt Position.

DUTIES AND RESPONSIBILITIES:

Collects, reviews, and submits monthly reports at the end of every month and files monthly reports as needed.

Makes monthly home visits (required) to ICWP individuals and makes phone contact on an as needed basis; sometimes will be required to call during off-hours, weekends, and holidays.

Conducts quarterly reviews to track medical status and care path goals and assists with completion of annual renewal services for caseload.

Maintains and monitors individuals' files and records by periodically reviewing and following up on those reviews.

Acquires medical information on individuals served and assists with medications as needed.

Provides quality services, consistent with the needs of the individual member, which are effective in developing, improving and maintaining the member's independence to live actively, safely and successfully in the community.

Monitors the overall health status and wellbeing of individuals to ensure that medical and social issues are addressed in a timely manner.

Assists the individuals with doctors' appointments and with following physicians' recommendations as needed.

Attends community/agency meetings regarding ICWP individuals as necessary.

Provides cost effective services to assist individuals in living as independently as possible in the home and community.

Collaborates with participating physicians and medical facilities to ensure wrap-around case management of individuals' health needs.

Involves the member or member's representative in the provision and decision-making process regarding member care.

Provides and coordinates related services to individuals as follows: transportation; setting up medical appointments; counseling; information and referral; emergency response systems; personal support; and medical supplies

Promotes and protects individual's rights, especially the right to freely make and express preferences in choices regarding their daily living and care as long as such rights/choices are not detrimental to the individual's well-being.

Demonstrates compassion for those served by treating members with dignity and respect while providing quality services.

Performs other duties as may be outlined in the Policies and Procedures manual for ICWP.

Other duties as assigned.

Qualifications: All case managers enrolling in the ICWP must be licensed as a registered nurse or have a BA or BS degree from an accredited college or university. Either the RN or bachelor's degree case manager must have a degree in a health care or human services related discipline and must minimum of two years demonstrated and documented experience in healthcare service delivery or human services case management. Experience must be pertinent to the conditions and disabilities of the populations served by the ICWP and include work with adults, the disabled, traumatic brain injured or other special needs populations. . Enhanced Case Manager qualifications include a R.N. or Bachelor of Science Degree in Nursing or Master's degree in social work; psychology; counseling or a related human services degree; a current certification in rehabilitation or case management; a license to practice social work, psychology, counseling, or related area in the State of Georgia and certification through the Georgia Medical Care Foundation to perform the Personal Support Consumer Directed Option.
