Summary

Provides and directs Physical Therapy services to individuals including assessment, treatment development, and implementation of programs and related professional services designed to alleviate pain, restore function, prevent disabilities, and promote overall fitness and health of patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and /or ability required.

  • Provides Physical Therapy services to patients and initiates referrals when appropriate and in accordance with Physical Therapy Department procedures.
  • Conducts screenings of residents to determine need for intervention/treatment.
  • Interprets and communicates evaluation findings to residents, family and other team members.
  • Establish schedules in conjunction with other team members.
  • Develops treatment plans by indentifying a problem list, including short and long-term goals and methods to achieve identified goals.
  • Coordinates treatment plan with resident, family and other team members.
  • Implements and conducts treatment as outlined in treatment plan.
  • Monitors resident’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
  • Participates in discharge planning.
  • Develops appropriate home or community programming to maintain and enhance patient performance.
  • Provides/recommends/fabricates adaptive devices or other equipment; trains patients and caregivers in the use or application of orthotics and prosthetics.
  • Documents results of resident’s assessment, treatment, follow-up, and termination of services.
  • Reviews the quality and appropriateness of the total services delivered and of the individual Physical Therapy programs for effectiveness and efficiency.
  • Supervises and directs PTAs in the implementation of the treatment plan.
  • Supervises students and volunteers in accordance with state regulations.
  • Supervises physical therapy tech/aides.
  • Participates in family, staff and patient education.
  • Attends in-service meetings as required.
  • Provides in-service education to the members of the staff and community.
  • Adheres to Physical Therapy Department procedures regarding documentation of Therapy services.
  • Consistently demonstrates safe judgment in evaluation, planning and implementation in the care and follow up of therapy programs.
  • Assumes responsibility for self-directed learning through continuing education and professional development.
  • Records billable treatment minutes according to Physical Therapy procedures.
  • Provides treatment in accordance with established standards of practice and department procedures.
  • Reports to work on time and adheres to schedule during assigned shift.
  • Maintains a safe and clean work area.
  • Complies with Infection Control, Universal Precautions and OSHA standards for the health care professional.
  • Maintains positive relationship and rapport with co-workers, patients, family members and facility personnel.
  • Assists with and participates in Quality Assurance Programs as required.
  • Adheres to established confidentiality standards.
  • Projects a positive and professional image at all times.
  • Practice the proper fire, safety and disaster procedures as set forth by SMC.
  • Participate in all quality, safety, security and infection control programs that are mandatory as well as those required and provided by the department.
  • Uphold confidentiality and HIPAA compliance at all times and report violations to the Privacy Officer.
  • Understand and comply with the requirements of the Saunders Medical Center’s Compliance Program, including, but not limited to the Code of Conduct, the Compliance Policy, all Saunders Medical Center-wide policies for compliance, and compliance plans affecting specific duties and responsibilities.
  • Participate in all education and training programs regarding compliance as required by Saunders Medical Center policy and as requested by the Supervisor.
  • Demonstrate personal traits of a high-level commitment, motivation and energy; team orientation; professionalism; trust; personal honesty and integrity; and a demonstration of placing others in a place of high value and treating them with dignity and respect.
  • Other duties as assigned.
Requirements

Must be able to work in excess of 40 hours per week when needed.
Must maintain appropriate professional license.
Must Maintain CPR/AED certification.
Must keep current on evaluation and treatment techniques.
Must be able to effectively motivate and direct staff to provide excellent care for SMC’s
patients
Must participate in continuing education to be able to provide high quality service
Must practice legally and ethically according to state, federal and APTA regulations and guidelines

Supervisory Responsibilities

This position has supervisory responsibilities of PTAs and physical therapy techs/aides.

Education and/or Experience

High School diploma
Masters or Doctorate Degree in Physical Therapy from an accredited program of physical therapy.
Licensed, or eligible for licensure in Nebraska.
Successful completion of National Certification examination for Registered Physical Therapist.
PC Skills – Ability to leverage various computer applications to carry out essential duties of the position. (Including, but not limited to DHS, MS Excel, MS Word, etc.)

Certificates Licenses, Registrations

Licensed in the State of Nebraska to practice Physical Therapy

Language Skills

Must be able to read, write and follow written and oral instructions and possess positive skills in communication. Ability to deal tactfully with personnel, patients, visitors, and the general public.

Mathematical Skills

Ability to calculate at the level necessary to perform essential duties of the position.

Reasoning Ability

Ability to interpret a variety of instructions furnished in oral, written, diagram or schedule form. Should be capable of following policies, interpreting statistics, making decisions, and performing marketing activities.

Physical Demands

Must be able to lift 70 pounds from floor to waist occasionally.
Capable of exerting 20 to 50 pounds of force occasionally (less than 1/3 of the time), and/or 10 to 20 pounds of force frequently (1/2 of the time), and/or up to 10 pounds of force constantly (2/3 or more of the time) to move objects, equipment, and/or patients.
While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, kneel, bend or crouch; use hands to manipulate tools, equipment, or controls; reach with hands and arms, balance, lift and perform medium to maximum transfer assists with residents. The employee is required to have visual and hearing acuity sufficient enough to assess resident safety and ability. The employee works mostly in the hospital facility in clinic or patient rooms.
