Job DescriptionJob Description

Responsibilities: 
•    Welcomes new residents to the facility, providing orientation to schedule, responsibilities, and goals of respite stay;
•    Help each resident identify goals for recovery during their stay;
•    Provide information about, and linkage to, community supports and resources in furtherance of personal goals and needs;
•    Provide direct assistance to address social determinants of health, i.e., access to food stamps, housing applications, transportation, and other benefits, advocacy for consumers involved in criminal legal system, etc.;
•    Provide practical help and support, advocacy and care coordination in partnership with the individual resident and any care team members;
•    When appropriate, accompany residents to medical, clinical, benefit, court, or other appointments related to individual recovery needs;
•    Provide encouragement and support for individuals to adhere to any prescribed medication regimens;
•    Utilize personal experience to help residents self-manage recovery, providing coaching/role modeling;
•    Follow up with residents who have discharged from the program to offer any additional support with transition and recovery plan;
