High School diploma or GED; valid driver’s license required; Background Checks and Drug Screens are required; must be able to express thoughts in writing; must be able to accept responsibility with minimal supervision; able to understand and implement written policies and verbal instructions; experience with disabled adults preferred; must be able to lift 30 lbs.; must be able to certify in MAC; CPR and First Aid; CPI certified. Shifts may vary based on agency need: 7:00 a.m. to 3:00 p.m.; 3:00 p.m. to 11:00 p.m.; 11:00 p.m. to 7:00 a.m. Must work some weekends. Must plan to work holidays as assigned; able to attend some evening meetings.
ESSENTIAL FUNCTIONS:
- Responsible for following SCLH activities:
- Reliably assess service needs of residents and report such to Residential Coordinator;
- Participate in development and implementation of resident’s Individual Support Plan (ISP);
- Assist clients in menu selection and preparation of balanced meals;
- Prepare, with clients, necessary food lists and submit for approval to Residential Coordinator;
- Insure clients maintain house and grounds to comply with appropriate standards and general cleanliness;
- Ensure adequate inventories are available for weekend or other relief staff;
- Develop and maintain positive relationships with residents, staff, neighbors, resident families and the public.
- Responsible for providing direct services to residents:
- Will contact on-call supervisor or appropriate staff to insure proper emergency medical care is provided when needed;
- Identify and respond appropriately in a timely manner to needs and requests of residents;
- Provide individual instruction and training in accordance to the ISP (i.e., training with food preparation, physically assisting residents with household upkeep, bathing, laundry, etc.);
- Ensure safety of residents and immediately report hazardous conditions to appropriate staff;
- Administer and document administering prescribed medication, as required with zero error tolerance; errors are to be reported immediately to MAS Nurse.
- Responsible for the completion and/or maintenance of required reports:
- Complete and submit all required accident/incident and other reports on a timely basis;
- Complete and insure timely entries to online database (THERAP);
- Document all training;
- Maintain strict confidentiality regarding all resident information.
- Transport residents to functions, appointments or recreational activities in agency vehicles *ONLY IF INSURABLE TO DRIVE UNDER AWE/Arc INSURANCE*
a. To emergency room as directed;
b. Properly operate vehicle in compliance with AWE/Arc policies and state and local laws;
c. Ensure that all passengers conform to safety and personal conduct policies;
d. Submit all maintenance/repair requests to transportation supervisor.
- Responsible for attending required in-services and other staff meetings.
- From time to time will perform additional tasks in support of the SCLH or its residents.
