Job DescriptionJob Description

We are seeking a dedicated Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide psychiatric evaluations and medication management services. This role involves working collaboratively with the nursing home’s counseling services to address behavioral health issues in adult clients. The PMHNP will have external supervision by a psychiatrist under a Collaborative Practice Agreement and will have the opportunity to expand into telehealth services.

Essential Duties and Responsibilities:

  • Clinical Services: Deliver outpatient psychiatric evaluations and medication management. Assess, diagnose, and treat adults with behavioral health disorders.
  • Medication Management: Prescribe, recommend, or discontinue medications and treatments as per the Collaborative Practice Agreement and state regulations.
  • Collaboration: Work with counselors, social workers, and other treatment team members to ensure comprehensive client care.
  • Documentation: Accurately complete medical records, including assessments, treatment plans, progress notes, and billing entries. Maintain at least 10% of records for biweekly review by the supervising psychiatrist.
  • Communication: Counsel clients as necessary, design individualized treatment plans, and collaborate with family members and guardians on treatment plans.
  • Diagnostic Testing: Order and review laboratory or diagnostic tests to assess general physical condition and mental disorders.
  • Client Interaction: Return calls and emails promptly (within 24 hours or sooner if clinically necessary), maintain positive relationships with agency staff and clients.
  • Education: Participate in agency and department education and training to stay current with clinical policies and procedures. Provide education to clinical staff when requested.
  • Quality Improvement: Seek ways to enhance and promote quality of care and services.

Job Requirements and Qualifications:

  • Education: Master’s or Doctorate Degree in Nursing with a focus on Psychiatric Nursing; graduation from a state-certified advanced practice program specializing in psychiatry/mental health.
  • Experience: Minimum 1-2 years of direct client care for psychiatric disorders, with at least 3 years of experience as a psychiatric provider. Experience in telemedicine is a plus.
  • Licenses/Certifications:
    • Certification by the American Nurses Credentialing Center (ANCC) in Psychiatric Mental Health (PMH) as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
    • Unrestricted license to practice as an APRN in Illinois.
    • Valid DEA number and unrestricted DEA license in Illinois.
    • State-issued Controlled Substance Registration (CSR) and registration with the state Prescription Monitoring Program (PMP) in Illinois.
  • Skills:
    • Experience delivering high-quality, client-centered care.
    • Proficiency in EHR documentation and Microsoft Office Suite.
    • Ability to maintain client confidentiality and communicate effectively with diverse groups.
    • Strong organizational skills and the ability to work independently and as part of a multidisciplinary team.
    • Capability to make independent decisions and foster teamwork.

Working Conditions:

  • Hours: Full-time, 40 hours per week with flexibility between 7:00 am and 9:00 pm.
  • Direct Services: Must meet minimum required hours of direct client services each week; remaining hours for documentation, meetings, training, and supervision.
  • Environment: In-person services at the nursing home with potential growth into telehealth services.
