Job DescriptionJob Description
  • Provide PT and OT services to patients in accordance to the physician's plan of care.
  • Assess/ evaluate therapeutic/ rehabilitative/ functional status, & participation in the development of the total care plan; evaluate home environment for hazards or barriers to more independent living.
  • Initiates/ develops therapy program, establishes goals for potential rehab; instructs patients/caregivers on use of therapeutic appliances/ durable medical equipment.
  • Prepares, submits clinical and progress notes and summaries based on the attainment of goals.
