Early Head Start Health Service Advocate

The essential functions include, but are not limited to the following:
· Reviews and enters screenings, immunizations, and the rest of EHS-require forms into ChildPlus
· Follow-up on incomplete 30-day requirement, 45-day sensory screenings and 90-day health determinations
· Complete referral form when concerns identified on medical documents or intake forms.
· Follow-up with Health Service Manager in regard to health referrals
· Process documentation necessary when requesting medical records of all children in the
· Program
· Conduct initial and /or follow-up calls to medical providers regarding pending medical.
· Records
· Seek, maintain, and update health education materials, health insurance and clinic.

· Assist with staff trainings.
· Participate in conducting and assisting with self-assessment and community assessment.
· Demonstrate ability to work independently and as an effective member of a
· multidisciplinary team.
· Participate in Early Childhood & Family Engagement – EHS & other LAEP departments in
· staff meetings and trainings.
· Work flexible hours to meet the needs of families (e.g., evenings or Saturdays).
· Perform other duties as assigned
· Assist in planning and organizing the quarterly Health Services Advisory Committee (HSAC) with the community, EHS parents, and staff.
· Facilitate and advertise virtual or in-person health and nutrition education classes to Early Head Start parents and staff based on community needs.
· Collaborate with Early Childhood Educators and Teachers to review missing/expired health paperwork and medical/dental records.
· Partner and collaborate with neighboring organizations to create an effective resource and referral list.
· Meet with families with nutrition and health concerns to create goals and guidance.
Requirements:- Bachelor of Science in Nutritional Science or relevant certifications (ex. LVN, CAN, or Early Head Start Health Certification)
- Associate degree in health or nursing education or LVN or 5yrs. work experience, 3yrs. of experience with Early Head Start or Head Start.
