Responsibilities:

  • Diagnose and treat acute, episodic or chronic illnesses
  • Prescribe all necessary medications and treatments
  • Perform comprehensive physical examinations of patients
  • Develop and implement patient management policies and procedures
  • Facilitate referrals to other healthcare professionals and medical facilities
  • Communicate with collaborating physician or specialist regarding patient care
  • Order and interpret diagnostic and therapeutic tests relative to patient’s age-specific needs
  • Facilitate the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients
  • Complete a health screening/assessment for new patients and establish as needed
  • Document all clinical data as outlined in the organization’s policies and procedures. Keep documentation current. Responsible for typing progress notes, assessments and treatment plans

Qualifications:

  • Previous experience in nursing or other medical fields
  • Ability to build rapport with patients
  • Ability to thrive in a fast-paced environment
  • Excellent written and verbal communication skills
  • Strong leadership qualities
  • Graduate of an accredited educational program with a master’s degree in nursing or doctorate degree in nursing or physician assistant degree
  • Board certification must be current
  • Active and current DEA certificate without restriction
  • Proficiency with an Electronic Medical Record
  • Unrestricted license to practice within the State of Illinois
  • Bilingual (English/Spanish or English/Polish) preferred
