Job DescriptionJob Description

We are currently looking for a person to provide direct support to individuals with intellectual disabilities in a residential setting for a group home.

The ideal candidate should have experience as a caregiver or in the healthcare field and should have the ability to work independently, be flexible, patient, motivate others and exercise good judgment and make timely decisions.  They must be familiar with the Microsoft office suite and have a good understanding utilizing technology to document notes.   

Support is to be provided on a day-to-day basis with routine personal needs, social support, assistance in communication and physical assistance in a wide range of activities in order to reach their individual goals.

CPR/ First Aid, Medication and Therapeutic options training provided.  Associates degree or equivalent.

Key Responsibilities:

  • Assist and document the development of skills as indicated in individualized support plans created by the individual’s support team.
  • Assist individuals with daily routines (personal hygiene, shopping, cleaning, cooking, etc.) and community outings as needed.
  • Coordinate leisure activities and medical appointments for individuals.
  • Collect, record, submit and file accurate data on client attendance, implementation of individual support plans, and provided services.
  • Complete all required training to maintain licensing requirements and participate in training to develop professional skills as needed and requested.
  • Perform other duties as instructed or assigned by your supervisor.
  • Ability to use software to input daily development skills for individual(s).
