Job Summary:
- Updating patient specialty care plans
- Preparing patients and providers for upcoming visits
- Coordinating care with community partners
- Coordinating post-hospital plans of care
- Collaborating with the Interdisciplinary Care Team (IDC)
Nurse Care Manager Knowledge/Skills/Abilities:
- Strong computer skills
- Ability to multi-task at a very high level
- Strong problem-solving abilities
- Excellent phone/customer service skills
- Familiarity with EMR software
- Strong Clinical Background
- Ability and willingness to expand clinical knowledge
Required:
- Current Registered Nurse (RN)
- Minimum 3 years of clinical experience

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $39.00 - $45.00 hourly with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
