Job Description for Program Specialist

Fulfills all duties associated with Department of Human Services (DHS) 6400 regulations related to the role of Program Specialist including (but not limited to):

• Complete initial participant assessments and annual updates within prescribed timeframes.

• Develops, implements and evaluates services for intellectually disabled people by coordinating services for assigned Individuals; providing on site supervision to assigned Individuals and direct service workers; possessing a working knowledge of the Pennsylvania Public Welfare Codes under Chapters 6400. ​

• Complete and submits any required documentation within established time frames.

• Ensure all aspects of each participant’s assessment, ISP, quarterly reviews and monthly reports are completed in a timely fashion.

• Ensure consistency of information in each participant’s record.

• Write reports, assessments and other documents in a clear, concise manner with correct grammar and spelling.

• Establish and maintain effective communication with direct support professionals to ensure participants achieve their goals.

• Attend and participates in ISP meetings and quarterly review meetings.

• Make sure all regulatory requirements are met including: compliance with the Office of Dev. Programs (ODP) regulations, Health Insurance Portability & Accountability Act (HIPPA) and all local/state/federal regulations.

• Understand and implements applicable regulations.

• Maintain participant’s confidentiality; files, verbal communication, all written records

• Work with direct care workers to promote a positive work environment, valuing the strengths of everyone.

• Be willing to train as a Certified Investigator for ODP’s Incident Management Protocols.

• Be willing to take the training to become Medication Administration Trainer and Practicum Observer for Community Homes.

• Complete MAR reviews

• Comply with all policies and procedures, and uphold the mission, values and philosophy of Apex Healthcare services, llc.

• Supervise, mentor and counsel program staff and evaluate staff annually, or as needed.

• Assist in the interviewing and hiring of program staff.

• Attend staff development and training programs as required.

• Other duties as assigned, job description is summarized and is not all inclusive. At the discretion of Management, and according to circumstances, verbal/written additions or deletions may be made.

• Complete a minimum of 24 hours of training in the areas of Human Services and Program Planning and maintain other required training e.g CPR, First Aid etc
