Job Summary: The Intake Coordinator plays a crucial role in managing a caseload with multiple deadlines, ensuring compliance with state and federal regulations, and adhering to contract guidelines. The successful candidate will be responsible for responding to referrals, meeting with families and children, conducting developmental & hearing screenings, collecting demographic and medical information, and completing family assessments. This position also entails home visits to facilitate initial intake, eligibility determination, and the development of Individualized Family Service Plans (IFSPs). The Intake Coordinator will assemble a qualified multidisciplinary team for eligibility determination and assist parents of eligible children in accessing all services identified in the IFSP. Furthermore, this role involves ensuring that appropriate IFSP teams are established to determine suitable levels of services, aligning with the individual child and family needs identified during the assessment process. Effective management of required documentation, including timely sharing with IFSP team members, is a crucial aspect of this role.

Responsibilities:

  • Respond to referrals within 3 days of receipt
  • Complete the initial meeting within 10 business days of referral
  • Perform developmental & hearing screenings, conducting family assessments, and gathering demographic and medical information
  • Coordinate multidisciplinary teams and facilitate meetings for eligibility determination and the development of IFSP within 45 days of referral
  • Assist parents in accessing all services identified in the IFSP
  • Ensure appropriate IFSP teams are established and services meet individual child and family needs
  • Manage required documentation and share it with IFSP team members in a timely manner

Qualifications:

  • Bachelor's degree in a Human Services field is required
  • Excellent writing and communication skills
  • Local travel is required; possession of a valid Louisiana driver's license, auto insurance, and a reliable vehicle is a must (company vehicles available for most travel)
  • Ability to develop collaborative relationships with families, providers, and stakeholders
  • Strong team player with good decision-making capabilities
  • Knowledge of early childhood development is essential
  • Successful completion of a criminal background check is required prior to employment
