*RN's can be in the Jefferson, Johnson, Wyandotte, Douglas, Franklin, Atchison, Jackson, Nemaha, Brown, Doniphan, Leavenworth, and Miami County areas in Kansas*

RN Care Manager Details:
- Location: Remote Field Work position 50% remote 50% member home visits in the county the RN resides in. must reside in the Jefferson, Johnson, Wyandotte, Douglas, Franklin, Atchison, Jackson, Nemaha, Brown, Doniphan, Leavenworth, and Miami County areas in Kansas
- Schedule: Monday-Friday 8:00am-5:00pm CST, No OT
RN Care Manager Compensation
- The pay for this position is $36 - $42 per hour based on experience plus mileage reimbursement
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN Care Manager Qualifications:
- Active RN license in the state of OK
- Must have experience with Pediatrics
- Care/Case Management and Electronic Medical Record experience
- Must have solid computer and documentation skills
RN Care Manager Responsibilities:
- Check email and voice mail for important communication and member needs
- Communicate with providers as needed
- Address member needs as they arise
- Plan/schedule in-person visits with members
- Complete required monthly telephonic outreach to members
- Complete all necessary documentation in EMR
- Travel to member homes biannually and perform assessments, required documentation
- Attend required meetings
- Work across teams to solve member issues
