Responsibilities:
- Collaborate with other team personnel to achieve well-rounded care
- Provides skilled physical therapy in accordance with the physician’s plan of care
- Assists the physician in assessing the client’s functional level by applying appropriate tests and assists in the development and revision of the plan of care
- Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care
- Observes, records, and reports the client’s reaction to treatment and any change in the client’s condition to the physician and/or client care manager and other staff involved
- Evaluates, and regularly reevaluates, the rehabilitation needs of the client; initiates, develops, implements and makes necessary revisions of the client’s plan of care
- Conducts OASIS assessments as appropriate
- Supervises physical therapy assistants as assigned to license in accordance with licensing board requirements, payer requirements and agency policies
- Develops, prepares and maintains individualized client care progress records with accuracy, timeliness and according to policies
- Submits accurate documentation within 24 hours of visit
Qualifications:
- Previous experience in physical therapy
- Ability to handle physical workload
- Ability to build rapport with patients
- Excellent written and verbal communication skills
- Strong leadership qualities
Requirements:
- Licensed as a physical therapist in the state
- Minimum of 6 months experience as a physical therapist
