QUALIFICATIONS:

  • Valid RN license in GA (Required)
  • Valid Driver’s License (Required)
  • Minimum of 1 year of experience in case management or related field
  • Familiarity with hospice care principles and practices
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in electronic medical records systems
  • Understanding of HIPAA regulations and patient confidentiality guidelines

DUTIES AND RESPONSIBILITIES:

  • Provide case management services to patients in SNF, ALF, PCH and Pt's home
  • Assess patient needs and provide care as prescribed in the Plan of Care
  • Coordinate and collaborate with interdisciplinary healthcare team members
  • Monitor patient progress and adjust care plans as necessary
  • Conduct regular patient assessments and evaluations
  • Educate patients and their families on disease management and options
  • Facilitate communication between patients and healthcare providers
  • Ensure compliance with HIPAA regulations and maintain confidentiality of patient information
  • Complete evaluations, assessments, and treatments in accordance with policy and the care plan
  • Participate in the on-call rotation
  • Complete all documentation and records in a timely and accurate manner
  • Follow established policies and procedures as related to the scope of practice
  • Attend and participate in bi-weekly Interdisciplinary Team Meeting (IDT)
  • Maintain continuity of patient care in a hospice setting
