Job DescriptionJob DescriptionSadler Health Center is recruiting a Community Health Worker to join our team.


The focus of Sadler’s Community Health Workers is to provide support to our patients and their families to establish primary care for health, dental, vision, behavioral health and assist patients and navigate systems to obtain assistance or resources when needed.

As a Community Health Worker, your organization and customer service skills will help you to independently establish, build and maintain trusting working relationships with patients and their families in order to assess and identify need or eligibility for appropriate programs.

Your experiences with medical terminology and customer service will assist in communication between providers, patients and involved loved ones to ensure service needs are achieved.

Sadler Community Health Worker team represents Sadler Health to our community and our patients with superior communication skills via phone interaction and face-to-face visits, and are skilled at remaining calm in high pressure or emergency situations.

We offer a comprehensive compensation and benefits package and starting pay for this role is between $18-22.

High School Diploma or GED and 6 months experience working with community health required, in addition to ability to work a flexible schedule and travel within our service area of Perry and Cumberland counties.
