Job DescriptionJob Description

JOB DESCRIPTION:     

Working under the direction of the Physical Therapist, provides therapeutic care, in accordance with the agency policy and procedure, plan of care and physician’s orders, to an individual in his/her place of residence or facility.

QUALIFICATIONS:               

Licensed physical Therapist Assistant with a current New Jersey license in good standing. Minimum of at least one year of professional experience as a PT assistant, preferably in a home-based environment or community outreach program. The experience must be in the United States or its possessions. Computer skills are necessary

Duties:

 

1.    Performs problem identification and goal setting.

2.    Develops, updates and revises and plan of treatment in collaboration with the Physical Therapist.

3.    Provides physical therapy in accordance with the plan of treatment

4.    Participates in care conferences and collaborates with other team and family members regarding treatment and changes in client’s treatment.

5.    Participates in client/family education as appropriate.

6.    Requisitions and manages equipment and supplies required to increase the client’s function and independence as appropriate to care.

7.    Follow proper procedures for Infection Control and Exposure Control.

8.    Maintains accurate and timely report and records, recording evaluation data, treatments, client’s response to therapy interventions.

9.    Participation in quality and performance improvement, and safety measures.

10.  Participation in in-service programs as required by the agency.

11.  Able to stoop, bend, lift and transfer patients. Carry bundles up stairs weighing up to 10 pounds, climb stairs and travel to and from prospective patient’s place of residence.

12.  Wear agency badge or identification during all work hours.
