Position Summary

Participate in coordinating operational activities of UICH. Assure compliance with policies and procedures and all applicable laws, regulations, and standards. Maintain records and security for controlled substances. Provide clinical pharmacy services to home infusion patients. Assessment, preparation of care plans, and monitoring response to therapy. Education of patients, caregivers, and other health care providers. Contacts patients and caregivers to assess their status and provide information. Review of laboratory results and other relevant clinical data to identify situations that require further follow-up. Follow-up with prescriber and/or other healthcare providers to discuss patients, provide information, and/or recommend a course of action. Documentation of clinical activities and outcomes. Monitoring and reporting adverse drug reactions and medication errors. Maintenance of patient medication profiles and drug interaction checking. Coordinate home infusion therapy with the patient care team (referral physicians, pharmacists, nurses) and home infusion staff.

Position Responsibilities

  • Enter home infusion orders into the computer system and determine the appropriate drug delivery method considering drug stability, patient needs, and other issues.
  • Checks products prepared by pharmacy technicians.
  • Assist with ordering drugs and supplies.
  • Communicatees necessary information to IV Admixture staff to ensure appropriate preparation of homecare products after hours.
  • Provide backup for absences of the Home Health Care Specialist.
  • Participate in the preparation of weekly compounding schedule
  • Participates in the orientation of new staff as appropriate.

Education Requirements

  • Pharm. D. (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy and one year of applicable experience is required.
  • Current licensure by the Iowa Board of Pharmacy Examiners is required.

Experience Requirements

  • Excellent written and verbal communication skills are required.
  • 3 years of experience in pharmacy is required
  • Job-related experience with and/or commitment to service excellence is required.
  • Job-related experience with and/or commitment to diversity in the work environment is required.

Desired Qualifications

  • Home Infusion pharmacy experience and clinical pharmacy experience are desirable.
  • Job-related experience with sterile compounding is highly desirable.
