JOB DESCRIPTION

We are a non-medical Senior Home Care organization currently experiencing significant growth ~~We truly love what we do and we're looking for a special LPN who also has the heart for enhancing the lives of the elderly & infirm as we do.

As the LPN, you will be responsible for developing care plans for our clients. You will also be responsible for the in home evaluations of our potential and existing clients. The plan of care that you develop for our clients is what our caregivers will adhere to. You will leverage your nursing degree, home health experience and your personal maturity, flexibility and responsibility to perform at a high rate of activity. You will teach the Palliative Care training program we have prepared for our caregivers. This position isn't for the faint of heart or anxious; if you desire responsibility and something new every day, then this is the position for you. In exchange for your dedication to our organization our caregivers and our clients, we offer:

a Healthy Part Time hourly pay (with 34 hours you can get full health insurance benefits), an excellent staff of 4(four) to back you up. a company car with gas included to make your rounds & see our clients and caregivers - meet and get to know them as you are part of a great team to help them stay independent in their home, an annual bonus of up to 10% of your pay,

Some of the responsibilities associated with this position are:

•Responsible for the direction and coordination of the home care activities. Inclusive of assessments, interviewing, training, and assisting the team in placing the right candidates in the client's homes, visiting our referral sources.

•Oversee services rendered by Certified Nursing Assistant and experienced Caregivers

•Conduct an assessment with patient and/or family and facilities or hospital they reside in

•Develop a Plan of Care for the client

•Ensure that an in-person orientation to the tasks being delegated will be provided to an assigned CNA or Caregiver

~Responsibility of scheduling the next plan of care assessments and care notes as required by the state regulations

•Responsibility for communicating all new state regulations

•Assigning and participating in the rotation of weekly covered on-call phones optional for extra pay

 

JOB REQUIREMENTS


The ideal candidate for the position will meet the following position requirements:

•Have excellent inter-personal and communications skills. This includes the ability to effectively assess a client's and caregiver's situation or need and develop an appropriate response in a timely manner.

•Have excellent leadership and people skills.

•Be able to effectively relate to the needs of potential clients and caregiver's and present an appropriate solution.

•Be highly organized and able to multi-task effectively.

•Be able to effectively delegate and hold subordinate staff accountable.

•Be computer literate to include the ability to navigate on-line scheduling programs. Familiarity with MS Office programs is also important.
