Job Description:
Position Purpose:
Evaluates plans, directs and administers physical therapy modalities of treatment as prescribed by a licensed physician. Assists patients in reaching their maximum performance and level of functioning, while learning to live within the limits of their capabilities. Participates in operational aspects of the department, maintains performance improvement activities within the department, and participates in CQI Activities when requested.
Job Responsibilities:
- Practices and Policies Consistently comply with the client's practices, policies (e.g., attendance, patient safety), and procedures. Abides by ethical standards set in the the client's Commitment to Principles Handbook. Regulatory Requirements Completes and maintains all regulatory requirements including licensing and certification, and other mandatory training within established time frames. Submits documentation 2 weeks prior to evaluation or as appropriate.
- Requirements may consist of, but are not limited to TB Testing, Safety Test, Compliance Training, HIPAA Training, Licensing and Certification.
- Patient/Customer Satisfaction - This standard is mandatory for those employees who have direct patient contact and may only be measured using an entity, department, unit or specific question Press-Ganey score. A patient satisfaction or customer service standard may be developed for non-direct patient care employees at the manager's discretion.
- Documentation - Documentation is completed in a timely, complete, legible, concise and accurate manner. Documentation follows the client's guidelines, professional guidelines, and meets third-party payor and regulatory requirements. Maintains records pertinent to departmental operations. Completes all documentation elements as appropriate (Interdisciplinary Plan of Care, Education record, billing, discharge plan, and others).
- Patient care - Performs an appropriate assessment on all patients as related to the therapy requested and provides information and feedback and reassess as per policy. Assesses patient pain that interferes with optimal level of function or participation in rehabilitation; and provides intervention.
- Teamwork - Communicates patient care and departmental issues. Accepts change and demonstrates flexibility when asked regarding work schedules and assignments. Completes additional assignments in a timely manner. Offers suggestions for resolving operational issues within the department. Provides unit specific information to students, floats, and travelers/registry personnel. Meets productivity standards established by department leadership. Participates in any plan to improve customer service. Patient Satisfaction scores meet percentile ranking established by leadership.
- Treatment plans - Designs, implements an appropriate treatment plan and evaluates treatment effect, recommending changes to physician as needed. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate. Demonstrates knowledge and safe use of modalities, equipment and therapeutic procedures. Coordinates and directs patient care to ensure patients' needs are met and hospital policy is followed. Completes development plan and performance is satisfactory.
CA Certifications And Licensure:
- Cardiopulmonary Resuscitation (CPR) - Required Physical Therapist (PT) - Required
Education:
- New graduate acceptable. Graduate of an approved and accredited professional Physical Therapy school or program.
