The Human Service Case Specialist
· Interviews applicants, assesses need and determines eligibility;
· Identifies community resources when needed and develops appropriate plan in conjunction with clients;
· Assists clients with state benefit applications, SNAP NH Medicaid, APTD.
The position requires:
- Minimum of 2 years of case coordination/management experience in human services;
- Skill set that includes superior communication skills, ability to build rapport and write detailed case notes on each client;
- An ability to work independently, collect required documentation, establish priorities and manage the expectations of individuals in need.
- Be able to articulate reasons behind decision making and problem solving;
- Respond to all calls, manage conflict, provide timely follow up;
Case Specialist provides a full array of human service supports to individuals and families. There are part time positions available.
