Organization Overview: Meals on Wheels Orange County is the largest nonprofit provider of nutrition and supportive services for at-risk older adults in Central and North Orange County. We deliver nearly 2 million meals annually to 20,000 seniors, striving to combat hunger and loneliness through nutritious meals, friendly visits, and safety checks. Our services also include day care to help keep families together. Meals on Wheels Orange County operates under the DBA Community SeniorServ, Inc.

Location: Remote (with scheduled office days in Anaheim, CA)

Hours: Monday - Friday, 8:00 AM - 4:30 PM

Position Status: Full-Time, 40 hours per week

Compensation: $21.85 - $23.50 per hour (final compensation determined based on geographic location, skills, education, and experience; fringe benefits not included)


Job Summary: Under the supervision of the Case Manager Supervisor, the Case Manager is responsible for developing and implementing individualized care plans for frail, at-risk seniors (aged 60 and over) within the Orange County service area. The goal is to enhance client functioning and support independent living. The Case Manager will manage a caseload of 125 cases, with potential adjustments based on departmental needs.

Essential Duties and Responsibilities:

  • Conduct client-specific assessments to identify needs, problems, and goals.
  • Arrange and coordinate a network of supportive services and entitlements according to the care plan.
  • Monitor and evaluate the effectiveness of services in meeting long-term goals.
  • Budget and maintain records for homemaking and personal care services.
  • Coordinate homemaking and personal care services with contracted vendors.
  • Arrange and coordinate volunteer "chore" services.
  • Maintain required records and reports in compliance with all regulations.
  • Develop community resources and maintain a current directory of relevant services.
  • Represent the agency at county and agency meetings as needed.
  • Participate in team meetings to review client concerns, issues, and goals.
  • Screen incoming homebound meal referrals for potential case management clients.
  • Participate in outreach efforts.
  • Practice safe operating procedures and ensure a safe work environment.
  • Perform other relevant duties as assigned.
  • Use personal transportation for client care activities.

Education, Experience, and Qualifications:

  • Bachelor's Degree in Human Services, Sociology, Psychology, Social Work, or a related field.
  • Prior experience with frail, elderly individuals is preferred.
  • Sensitivity to and awareness of the needs and resources for this population.
  • Strong assessment and evaluation skills; knowledge of case management principles and techniques.
  • Ability to maintain professional and confidential standards.
  • Excellent communication skills, both verbal and written.
  • Ability to develop cooperative working relationships with community resources, clients, informal support sources, and colleagues.
  • Working knowledge of programs and services available for seniors in Orange County.
  • Must have personal transportation, a valid California Driver's License, and auto insurance as required by state law.
  • Must pass a post-offer background check.
  • Bilingual in Spanish or Vietnamese is preferred.

Physical Job Requirements:

  • This position operates in a fast-paced, multi-tasking, professional environment, requiring ongoing learning and complex problem solving.
  • Ability to sit for extended periods and use both hands and fingers for typing.
  • Ability to operate office equipment such as copiers, printers, filing cabinets, and telephones.
  • Occasionally lift and move office equipment and materials up to 10 pounds.
  • Frequent computer use requires close vision.
  • The noise level in the work environment is usually moderate.
  • Occasional overtime required.
  • Local travel required up to 50% of the time.
