CHILDREN’S SERVICES – Wraparound PROGRAM

Clinician

 

OVERALL RESPONSIBILITIES: Under the direction of the Clinical Program Supervisor functions as Clinical Team Leader for children and youth in the SCHARP Wraparound program. Provides comprehensive, integrated, therapeutic mental health services to all assigned clients. The Clinical Team Leader is the supervisor of the assigned Wraparound staff team. Adheres to all required documentation standards.

REQUIREMENTS:

1. Must be able to pass California State Department of Justice criminal background investigation, Live Scan and/or fingerprinting.

 

EDUCATION:

· Master’s degree in Social Work, Marriage and Family Counseling, Psychology

· Licensed / Registered by the approving State of California Board by discipline as an LCSW, LMFT or PhD.

· Qualifies as a Licensed Practitioner of the Healing Arts (LPHA) by the Department of Mental Health.

 

EXPERIENCE:

· Must have at least three years’ experience working with mentally ill children.

· One year of management / supervisory experience preferred, but, not required.

 

SPECIAL SKILLS AND KNOWLEDGE:

· Ability to provide leadership to administrative and clinical staff.

· Ability to relate to individuals who have severe impairments in their ability to function in the community.

· Ability to respond quickly, effectively and with equanimity to client/family crises and life situations.

 

· All direct service personnel will be expected to maintain a flexible schedule (which will not be traditional 9-5) and must be available to provide services in the community/in home service as designated by the people who will be served within the Wraparound program.

 

· Knowledge of Los Angeles County Department of Mental Health’s Rehab Option model of service provision and documentation requirements.

 

· Basic computer skills and literacy are required to use the agency electronic health record. Staff must be able to logon, type, use a mouse and negotiate through various screens or windows to complete daily chart documentation.

 

· Ability to lead a team and work as a part of team both inside the agency and with other agencies, community resources and member’s families.

 

· Must have valid California Driver's license and the availability of a car with adequate insurance.

 

DUTIES:

· Meets weekly with the Wraparound Program, Clinical Program Supervisor. Keeps supervisor informed of all aspects of the assigned SCHARP Wraparound program Team in a timely manner.

 

· Supervises assigned Wraparound program team staff; Facilitator, Child & Family Specialist and Parent Partner to ensure that all receive an adequate orientation to their position, information, support and resources to carry out the duties of their job.

 

· Functions as therapist for children and youth in the SCHARP Wraparound Program

 

· Meets weekly with the assigned program staff providing supervision, assisting them in identifying barriers to achieving program goals and in developing solutions. Maintain log of all meetings that includes topic, staff signatures and materials reviewed or hand-outs.

 

· Follows the agency’s personnel policies when in the process of hiring, disciplining, and terminating staff. This includes the involvement of the Wraparound Clinical Supervisor and the Director of MHSA-FSP and Children’s Services Programs.

 

· Reviews and signs assigned staff timesheets, vacation/absence requests. Reviews assigned staffs check requests/ flex funds forms for appropriateness and need prior to submitting to Clinical Program Supervisor for approval. Complete critical incident reports and advises supervisor of clinical incidents.

 

· This position holds the role of being the Single Fixed Point of Responsibility (SFPR) for the youth and families served in the Wraparound program. The Clinical Team Leader is responsible for completing required SFPR tasks as defined by the Department of Mental Health.

 

· Functions as LPHA for assigned clients; completing the Children’s 8 page Initial Assessment, Client Care Coordination Plans and required updates, ensuring they are clinically relevant to the individual client as well as completed, reviewed and signed in a timely manner as specified by Los Angeles County Department of Mental Health standards. Completes all Outcomes documentation (CAFAS / CANS, and any others required) in a timely manner based on DCFS standards

 

· Works with program staff to coordinate and monitor DMH Client Care Coordination Plans, DCFS Plan of Care, Safety Plan and Crisis Plans to ensure services are coordinated and are reflective of the child and family’s needs, desires, goals and objectives. Reviews and approves the Plan of Care and Safety Plans. Ensure clients / family are involved in establishing goals in the areas of living arrangements, social relationships, age appropriate developmental tasks, education, and dental and medical care.

· Supervise and coordinate all aspects of incoming referrals / prescreening child /youth into Wraparound program services and disenrolling/discharging clients as appropriate to Wraparound program guidelines.

 

· Participates in the Quality Assurance and Utilization Review Committees, ensuring that all client cases are reviewed in a timely manner and that all documentation meets Los Angeles County Department of Children and Family Services and Department of Mental Health standards.

 

· Work as a team with Department of Children and Family Services (DCFS), Department of Probation, Foster Care system, psychiatrists and other interagency and intra-agency staff to provide consistent services.

 

· Provide crisis intervention services as scheduled on an on-call basis twenty-four hours, 7 days a week to members at risk of hospitalization, detention and /or incarceration in an effort to avert hospitalization, detention and /or incarceration.

 

· Maintain contact with family members and significant other support persons of clients as appropriate.

 

· Maintain a Daily Service Log on a daily basis, documenting all services to be billed and completing the progress notes for these services. Service codes and time billed must be consistent on the Daily Service Log and progress note. Documentation is to be completed and submitted in a timely manner. Under no circumstances should a billing sheet be submitted for data entry without the progress note completed.

 

· Involve clients / family in all aspects of the services they receive as partners, providing them with choices and with opportunities for empowerment whenever possible.

 

· Participate in CFT meetings minimally at critical junctures in client and family care. More often as practical and required to ensure quality services delivery. Attend all POC Review meetings with the DCFS ISC liaisons for the clients on the assigned team.

 

· Prepare for weekly clinical staff meetings and supervision by having current information on the status of all assigned clients and program assignments.

 

· Provide mental health therapeutic and rehabilitation services as needed by assigned clients.

 

· Other duties as assigned.
