We are currently seeking a motivated and compassionate Long Term Care Pharmacist to join our team in Carbondale, Illinois. This position offers the opportunity to make a significant difference in the lives of patients in long-term care facilities.

Job Description: As a Long Term Care Pharmacist with RPH on the Go, you will be responsible for managing medication therapy and improving pharmaceutical care for residents in long-term care settings. Your expertise will contribute to enhancing the quality of care through medication management, patient counseling, and collaboration with healthcare teams.

Responsibilities:

  • Review and manage medication therapy for efficacy, safety, and adherence.
  • Consult with physicians, nurses, and other healthcare professionals to optimize care plans.
  • Oversee the preparation and dispensing of medications.
  • Provide medication therapy management (MTM) services, including comprehensive medication reviews and targeted interventions.
  • Educate residents and their families on medication regimens, potential side effects, and interactions.
  • Implement and maintain policies and procedures that ensure compliance with regulatory standards.
  • Participate in quality improvement initiatives aimed at enhancing patient care.

  • Pharmacy Operating Hours:
  • Monday - Friday: 7:00 AM - 8:00 PM
  • Saturday - Sunday: 9:00 AM - 5:00 PM

Requirements:

  • IL Pharmacist license
  • Hospital or LTC experience
