Job Summary:

We are seeking a dedicated Health Educator to join our team. The ideal candidate will have a passion for public health and educating individuals on various health-related topics. This role involves promoting healthy behaviors, conducting educational programs, and providing resources to improve overall well-being.

Primary Responsibilities:

· Provide initial staff development and training and periodically cover care plan development, prioritizing educational needs of clients; case conferencing; and follow-up. Orient new staff to program.

· Select and/or produce culturally competent health education materials, including audiovisual aids, suitable for the client group and maintain an annotated list of these materials for the staff to use in selecting materials for individual clients based on each client's education care plan.

· Train staff for group and individual education of clients.

· Plan and coordinates in-service education programs for patients and/or staff dealing with prevention of disease, family planning and Cancer Detection Program (CDP).

· Provide consultation and technical assistance to the provider and staff about prioritizing individual clients' educational needs, principles of adult learning, educational methodologies and materials, and educational barriers.

· Establish and annually update local resources and referral network for Perinatal services which are not available from the provider, e.g., smoking cessation programs, childbirth education classes, hospital tours, parenting support groups, adolescent pregnancy services, dental, and substance abuse programs, etc.

· Develop and implement a quality assurance and evaluation plan for the health education component of CPSP services.

· Provide direct client education services as needed and supervision of and /or consultation to staff performing health education to high-risk clients.

· Develop and teach use of client education materials to the staff.

· Plan, develop and evaluate educational programs to meet the health problems of the patient population and the community.

· Provide prenatal patients with Comprehensive Perinatal Services Program (CPSP) Assessments.

· Provide follow-up CPSP Reassessments to prenatal patients and additional follow-up visits for high-risk patients.

· Complete referrals of high-risk patients to local community agencies.

· Prepare order and distribute brochures, booklets, posters, audio visual materials to be used in health education.

· Attend meetings, trainings and conferences relevant to job description.

· Provide health education counseling for patients dealing with specific health conditions such as pregnancy induced hypertension, gestational diabetes, sickle cell disease, sexually transmitted infections, etc.

· Provide lectures to schools, various community groups, public/private agencies concerning health related issues and to promote services offered at WHCC.

· Assist with planning and organizing of annual events for prenatal and postpartum patients.

· Daily completion and submission of encounter forms for Health Education services.

· Submission of monthly reports to the department head which address accomplishment of department objectives, activities, obstacles, if any and highlights of the month.

· Follow-up of Health Education broken appointments.

· Assist in the annual review and revision of the Maternal and Child Health Department Protocols.

· May supervise other para-professional staff.

· Attend monthly case conferences as required by CPSP program.

· May perform other duties as assigned.

Key Requirements:

· A Bachelor's degree in Health Education.

· 3 years’ experience developing and implementing educational programs for patients in an acute care hospital or ambulatory setting.
