Job DescriptionJob DescriptionWe are looking for a Home Care Coordinator to add to our team

Plans and coordinates shift care for home health service referrals by: communicating with patients/family members regarding service needs, assisting with recruitment of nursing staff, scheduling nursing staff for individual cases based on patients needs.

Responsibilities and Duties:

  • Collaborates with other members of this organization to develop and implement staffing plan.
  • Assembles and maintains monthly staffing schedules for individual shift care cases based on plan of care.
  • Acts as liaison between clinical staff, patient/family, and office staff as appropriate, which may include answering after hour calls.
  • Recruiting nursing staff.
Qualifications and Skills:

  • Medical Assistant(preferred)
  • 1+ yr experience in healthcare setting.
  • Home Health or Clinical experience
  • Bilingual(preferred)
  • Demonstrates an ability to work with other health care clinicians as well as clinical and office staff.
  • Self directed and able to work with minimal supervision.
