Independence Home Health is currently seeking qualified LPN to join our team in Broward County. This position focuses on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
RESPONSIBILITIES:
- Develops and implements individualized care plans for each patient, in collaboration with their families and healthcare team
- Examines patients, and records health information and vitals
- Conducts on-going patient care assessments
- Administers prescribed medication, treatments, and therapies
- Monitors and evaluates patient response to treatment, medication, and healing progress
- Educates family members on patient clinical care to enhance positive outcomes
- May deliver specialty treatments and therapies
- Adjusts the child's care plan on an as needed basis
- Executes preventative initiatives to protect quality of care for patients
- Strong familiarity infection control procedures
QUALIFICATIONS AND REQUIREMENTS:
- Minimum of two years of pediatric LPN experience in a home setting
- Graduate of an accredited school
- Active Florida LPN/LVN license
- Commit to both part-time and per diem schedule
- Ability to work weekdays, weekends and some holidays as needed
Must be able to regularly travel the patients homes
Hold a current and valid CPR certification
KNOWLEDGE, SKILLS, AND ABILITIES
- Strong analytical and problem-solving skills
- Ability to prioritize tasks and delegate them when appropriate
- Ability to act with integrity, professionalism, and confidentiality
- Compassionate to patients and their families
- Ability to work well under pressure and/or stress and with time constraints
- Excellent verbal and written communication skills
- Excellent organizational skills and attention to detail
