Job DescriptionJob Description
  • Provide direct services to individuals, including linkage to resources to aid their immediate and future needs, advocating, supporting, and providing education as needed.
  • Treat individuals with Dignity and Respect.
  • Prepare sessions using goals and objectives listed on the ISP.
  • Complete all progress notes and quarterly reports using sound clinical documentation and abiding by all agency, Medicaid and licensure standards.
  • Respond to crisis situations within 1-2 hours.
  • Link individuals with wrap around providers and/or services, such as health services, social engagement activities, financial resources, employment resources, AA/NA groups to ensure their continuity of care.
  • Collaborate and coordinate with other service providers and professionals, such as case managers, social workers, and probation/parole officers.
  • Participate in individual supervision meetings with the direct supervisor as scheduled.
  • Attend all annual mandatory agency trainings (TOVA, First Aid & CPR, Human Rights, etc.).
