Job Summary: The Client Integration Supervisor is responsible for assisting individuals with disabilities in accessing and participating in community activities and opportunities. This role involves developing and implementing programs that foster inclusion, enhance social skills, and promote independence. The supervisor will work closely with clients, their families, and community organizations to create a supportive and inclusive environment. The supervisor will lead and support a team of Community Integration Specialists.

 

Key Responsibilities: 

  1. Client Assessment and Planning:

    1. Conduct assessments to understand the needs, preferences, and abilities of clients.

    2. Develop individualized community integration plans tailored to each client’s goals. 

  2. Program Development and Implementation: 

    1. Assist in the implementation of programs and activities that encourage socialization, skill development, and community involvement. 

    2. Coordinate with community organizations to create inclusive opportunities for clients.

  3. Support and Advocacy: 

    1. Provide direct support to clients during community activities, ensuring their safety and well-being. 

    2. Advocate for clients’ rights and access to community resources and services. 

  4. Skill Building: 

    1. Teach and reinforce social, communication, and life skills to enhance clients’ independence and confidence.

    2.  Provide training on using public transportation, accessing community resources, and participating in social activities. 

  5. Collaboration and Networking: 

    1. Establish and maintain relationships with local businesses, service providers, and community organizations.

    2. Collaborate with clients’ families, caregivers, and other support staff to ensure cohesive support.

  6. Documentation and Reporting: 

    1. Maintain accurate records of clients’ progress, activities, and any incidents. 

    2. Prepare reports and communicate regularly with supervisors and relevant stakeholders. 

  7. Training and Development: 

    1. Participate in ongoing training to stay current with best practices in disability support and community integration. 

    2. Provide training and support to volunteers and other staff members as needed. 

  8. Staff Training & Support 

    1. Assist in supporting, encouraging and training Client Support Team Members. 

  9. Additional duties as identified.

 

Qualifications:

  • High school education and Bachelor's Degree in related fields is preferred.

  • Previous experience working with individuals with disabilities.

  • Strong understanding of disability rights and inclusion principles.

  • Excellent communication, interpersonal, and organizational skills. 

  • Ability to work independently and as part of a team. 

  • Patience, empathy, and a passion for promoting inclusivity and empowerment. Working Conditions: 

  • The role may involve working evenings, weekends, and holidays to accommodate clients’ schedules and community events. 

  • Physical activity may be required, including assisting clients with mobility and participating in various community activities. 

  • Reliable Transportation 

    • Travel within the community is a significant part of the job.  

    • Punctual and Flexible
