Job Summary: The Clinical Manager provides oversight of all patient care services and personnel. Oversight must include the following: making patient and personnel assignments, coordinating patient care, coordinating referrals, ensuring that patient needs are continually assessed, and ensuring the development, implementation, and updates of the individualized plan of care. Also is a professional staff who is accountable for maintaining the quality of patient care by the agency objectives and policies, through planning, coordination, implementing, and evaluating the home health services provided. Lines of authority and reporting responsibilities: Reports to the Director of Nursing (DON). An employee in this class is “on call” for emergencies on a 24-hour basis.

Supervisory Responsibility: This position supervises Clinical Case Managers, RNs, LPNs, CNAs, Therapists, and HHAs as necessary.

Qualifications/Educational Requirements:

  • Registered Nurse in good standing with the Florida Board of Nursing.
  • Diploma, Associate, or Bachelor's degree in Nursing from a state-accredited RN program.
  • Valid Driver's license and acceptable MVR.
  • Demonstrate knowledge and skills necessary to provide care primarily to the geriatric population, and to a lesser degree, the pediatric and adult population.
  • Professional demeanor and appearance.
  • Excellent oral and written communication in English.
  • Demonstrates organizational and leadership skills.
  • Minimum 1-year supervisory experience.

Preferences: Minimum of two years experience in home health care with leadership or management experience.

Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

  • Coordinates and schedules the task assignments utilizing agency personnel to their fullest capacity.
  • Provides contact supervision of field nurses, other healthcare professionals, and home health aides to assure the quality of services and maintenance of standards.
  • Review and investigate the clinical content, diagnosis, medical history, medications, fee status, and other procedures of the record of patients under their supervision
  • Maintain compliance with company policies and procedures, laws and regulations, and professional standards within the state of practice.
  • Promote and manage expectations and satisfaction with internal and external customers.
  • Directs staff toward the appropriate use of community resources.
  • Appraises performance at probation and annually with the DON.
  • Provides the teaching and counseling necessary to enhance the delivery of services and professional development of nursing, professional, and health aide staff.
  • Sees patients in the home setting as needed.
  • Participate in on-call duties for after-hours client care on a clinical rotation.
  • Participates in audits, interdisciplinary team conferences, and the QAPI program.
  • Submits yearly self-evaluations goals, and objectives to the DON.
  • Reviews admissions and discharges.
  • Conducts weekly team meetings and submits reports.
  • Ensure HIPAA guidelines and procedures are maintained.
