Elevation Rehab is an in-house Outpatient rehabilitation service, providing care to Durham Ridge Assisted Living and Memory Care.
Elevation is seeking an in-house
Licensed Physical Therapist, responsible for the ongoing evaluation and application of therapy programs including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. Assists in developing and implementing new and ongoing programs beneficial for the enhancement of the resident care.
Licensed Physical Therapist Responsibilities:
- Carry out individualized therapy programs including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals.
- Physical therapists assess, treat, plan, implement, educate, and communicate to maximize a patient's progress toward achieving functional goals for all patients.
- Perform patient assessments and outline appropriate treatment programs for current patients and new patients of Elevation Rehab.
- Coordinate individual treatment programs with other clinical/therapeutic services as needed.
- Develop and track comprehensive goals for treatment to maximize patient outcomes.
- Deliver established therapy services and modify the care plan per the patient’s needs.
- Serve as an educator and point of contact for the in-house provider group, and any necessary medical providers to encourage continuity of care.
- Communicate with patients' family, facility staff and administration to assure your treatment plan is being followed and goals are being met.
- Maintain records utilizing our Electronic Medical Records system of all services and evaluations provided.
- Supervise therapist assistants and students as needed.
Licensed Physical Therapy Qualifications:
- North Carolina Physical Therapy License
- All certifications and licensing upon hire and must maintain current throughout employment.
Licensed Physical Therapist Education and Work Experience
- Bachelor of Science, Masters of Science Degree in Physical Therapy, or Doctorate in Physical Therapy
- Licensed by the State of North Carolina
- Experience in assisted living, skilled care, or memory care preferred.
Pay is based on a per visit rate not hourly.
