Job DescriptionJob DescriptionMinimum 1+ years pharmacy experience required.

We are currently seeking a qualified and dedicated Home Infusion Pharmacist to join our team for a 13-week contract in Northville, Michigan. The ideal candidate will be responsible for preparing and dispensing medications to patients in a home care setting, ensuring the safe and effective use of pharmaceutical drugs. This role involves working closely with healthcare professionals to customize treatment plans and ensure patient needs are met efficiently and compassionately.

Responsibilities:

  • Prepare, dispense, and manage medications to support patient treatment plans.
  • Collaborate with healthcare providers to assess, monitor, and modify patient care plans.
  • Educate patients and caregivers on medication administration, potential side effects, and medication management.
  • Ensure compliance with all health and safety regulations regarding home infusion therapy.
  • Monitor the effectiveness of drug therapies and report any adverse reactions or drug interactions.
  • Manage pharmaceutical supplies and maintain inventory to ensure adequate resources for patient needs.
  • Utilize sterile techniques for compounding and preparing medications as required.
  • Maintain accurate patient records and documentation for all clinical activities.

Shift:

  • Monday Friday: 8:00am 4:30pm or 8:30am 5:00pm

Requirements:

  • Home Infusion or inpatient hospital experience.
  • Experience working with IV compounding required.
  • Previous experience with a parenteral drug therapy admixture program and some clinical monitoring experience are preferred.
  • MI Pharmacist license.
