We are seeking a Care Manager/Medical Assistant to become a part of our team You will coordinate medical and health services for patients between medical visits in an outpatient  private medical  practice

Responsibilities:

  • Coordinate remote patient monitoring and other care management services with medical provider 
  • Duties include patient intake, coordination of care, patient education, gaps in care closure
  • Work collaboratively with provicers to implement lifestyle management programs 
  • Maintain excel dashboards and document progress of program participants 
  • Back up medical assistants with patient intake , teaching and medical care services including EKGs, CGMS etc

​Qualifications:

  • Previous experience in patient care management preferred 
  • Experience as a medical assistant preferred
  • Strong organizational skills
  • Excellent written and verbal communications skills 
  • EHR proficiciency ( EPIC preferred)
  • Knowledge of diabetes and obesity care desirable
