Pride Health is hiring a Nurse Practitioner in Winnebago County and Waupaca, WI

3-month contract role on-site at three Senior Living locations in Winnebago and Waupaca Counties.

Shift: Mon-Fri, 8am-5pm.

Pay: $70/Hour

Job Responsibilities:

  • Travel between 3 Senior Living facilities during shift
  • Conduct comprehensive, patient-facing assessments
  • Effectively manage acute and chronic medical and behavioral conditions
  • Ensure accurate and complete documentation, including ICD-10 conditions.
  • Conduct advanced illness and advanced care planning conversations
  • Provide patients and caregivers with counseling and education

Qualifications:

  • Active WI NP license
  • Basic Life Support (BLS) Certification
  • 1+ year of clinical experience in a long-term care setting
  • Proficient computer skills including the ability to document medical information with written and electronic medical records
  • Understanding of geriatrics, chronic illness, and advanced illness
