As a Clinical Pharmacist, you will play a crucial role in optimizing medication therapy management and ensuring the highest standards of pharmaceutical care. Your responsibilities will include:
- Conducting patient assessments to determine medication therapy needs.
- Collaborating with healthcare providers to develop and implement patient-specific treatment plans.
- Monitoring and adjusting medication regimens to achieve therapeutic goals.
- Providing drug information and education to patients and healthcare professionals.
- Participating in interdisciplinary rounds and healthcare team meetings.
Qualifications: To be successful in this role, you should possess the following qualifications:
- Minimum Education
- BS in Pharmacy or PharmD.
- Current Texas state pharmacy license in good standing.
- Board Certification in Pharmacotherapy (BCPS) or other relevant certifications preferred.
- Strong clinical knowledge and understanding of evidence-based medicine. ( open to new grads)
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a team-oriented environment.
- Active License as registered pharmacist in the state of Texas ( if re
Experience:
- Minimum of one year of clinical pharmacy experience.
- Experience in an acute care or hospital setting is preferred.
