Assist in setting challenging goals, tailor management and leadership efforts to achieve desired results by monitoring and evaluating departmental results related to clinical and business processes; update programs and processes to ensure future results compare favorably with relevant regional and national benchmarks

Provide the Hospice department with the management and leadership necessary to maintain a consistently high level of productivity, customer satisfaction, and clinical outcomes

Demonstrate, promote, and facilitate outstanding customer service; address escalated or unusual customer service issues in a timely and effective manner

Maintain awareness and demonstrates working knowledge of industry standards, laws, and regulations affecting or those which could potentially affect the department; provide recommendation and implement changes to ensure compliance with requirements; disseminate such information to team members, as appropriate in the interpretation of regulatory and accreditation guidelines and standards; ensure team member compliance with applicable laws, regulations, guidelines, and standards

Assist in guiding and directing team members in the application of CMS, Joint Commission, OSHA, MIOSHA, CDC and third party regulations; monitors adherence to those regulations

Develop and implement department and/or company policies to improve efficiency, improve performance, and reduce risk

Disseminate and interpret company policies and procedure to Hospice team members; provide guidance and training, as necessary; make suggestions for policy updates and process improvement

Pursue opportunities for new business by educating health care providers, potential clients, and the community on the company’s services and programs; collaborate with Business Development to facilitate the delivery of educational/promotional presentations and materials

Develop and maintain positive working relationships with referral sources, community members, partner organizations, corporate contacts, and vendors

Assist Hospice team members in the planning and delivery of care, achieving results that produce a consistent and high level of performance through case management activities, clinical practice and quality improvement

Assists the Clinicians with overseeing the direction of the interdisciplinary team in the provision of efficient, effective hospice delivery, promoting staff continuity and resulting in optimal patient satisfaction as measured by PAT/STAT

Promote timely, effective, and open communication between team members, patients, families, caregivers, physicians, and members of the IDT

In coordination with the CEO, plan and facilitate the department’s bi-weekly Interdisciplinary Team (IDT) meetings

Promote strategic planning in the daily scheduling of work assignments to ensure the needs of patients, families, and caregivers are consistently met

Assist in the management and maintenance of all Hospice department’s records, including the Electronic Medical Records system; ensure team members are completing required documentation in a timely, accurate, and compliant manner

Assist in executing and managing departmental and company-wide quality assurance, performance improvement, and risk management programs

Assist with the on-going evaluation of the Hospice team member’s professional and educational development by:

providing constructive feedback, counseling, and mentoring

identifying training needs

managing performance improvement processes

measuring program effectiveness and making updates, as necessary

Develop and facilitate team member training programs and sessions which:

enhance clinical competency

increase industry knowledge

ensure compliance with regulatory, accreditation, and reimbursement requirements

ensure compliance with departmental and company policies, procedures, and quality standards

ensure team members are knowledgeable of new or changing programs and services

Assist in the recruitment, selection, and onboarding of new team members; evaluate labor utilization and efficiency of the Hospice team; communicate recruitment and team member assignment recommendations to the CEO, as appropriate

Monitor team member’s work assignment to ensure weekly productivity standards are met

Ensure timely completion of the department’s bi-weekly payroll processing; act as back up in submitting payroll in office manager’s absence

Conduct annual performance evaluations on assigned team members

Hold Hospice team members accountable by ensuring team members perform duties in accordance with company and third party policy and practices, applicable laws and regulations, and accreditation standards

Work closely with the CEO and other clinical managers to:

promote and improve each patient’s quality of care

proactively identify and address patient and team member issues and/or concerns

develop effective plans of action and resolutions

encourage and promote teamwork, positive attitudes, high morale, open communication, respect, professionalism, and teamwork

Compile, analyze, and report on department data, status, or other information, as requested by the CEO and other clinical mangers

Function as a field nurse during extreme peaks, PRN’s and/or emergencies

Function as a field nurse within the on-call/after-hours rotation, as necessary

Triage incoming calls; ensure the case manager is made aware by completing appropriate documentation

Maintain standards of practice and documentation that are consistent with Federal, State, Local, Medicare, and accreditation regulations and standards

Demonstrate flexibility while performing job duties to ensure the highest quality is service is provided

Utilize critical thinking skills while performing job duties; analyze work process and make suggestions for improvement

Maintain a thorough understanding and consistently adhere to all applicable department and company policies and procedures

Conduct job duties in a way which will reduce and avoid potential safety hazards

Demonstrate initiative and drive to promote personal and company success

Perform other duties as assigned by your supervisor or management team member

EDUCATION AND EXPERIENCE REQUIRED

Bachelor’s degree, or higher, with a major in Nursing from an accredited institution

Current Registered Nurse license

Current BCLS certification

3 - 5 years of leadership and team building experience

1 - 2 years of supervisory experience

1 - 2 years of recent Hospice experience, preferably within the Home Health Industry
