Overview of the Position

Under the direction of the Rehabilitation Supervisor, the Physical Therapist develops and implements a plan for providing physical rehabilitation and home therapeutic treatment, as well as education of patients, family members, and significant others.

ESSENTIAL FUNCTIONS OF THE JOB SUMMARY:

1. Develops the physical therapy treatment plan and provides skilled physical therapy services required by the plan.
2. Evaluates the patient's level of function by applying diagnostic procedures and develops/revises the physician's plan of treatment based on best practice principles and in accordance with all applicable licensure definitions and commonly accepted professional standards of practice.
3. Performs direct physical therapy treatments based upon assessments and treatment plan.
4. Educates and trains patients/family on patient's total therapy program including additional exercises and equipment needed to aid recovery.
5. Instructs family and other home care staff on the patient's total physical therapy program.
6. Documents assessments, clinical findings, plan of action and evaluation of patient outcomes, in approved format and on appropriate medical forms in a timely manner.

Qualifications and Requirements

  • Current licensure as a Physical Therapist in the State of California.
  • Graduation from a Physical Therapy curriculum approved by the American Physical Therapy Association.
  • A minimum of one year’s experience as a Physical Therapist is required. Experience as a Physical Therapist in home health is preferred.
  • Current CPR card required

Service areas are San Luis Obispo and Northern Santa Barbara Counties. Multiply positions available, Full time, Part time, Per diem, with weekend rotation.
