Are you a dedicated and certified Dietitian-Nutritionist passionate about providing top-quality nutritional care in a Post-Acute Care (Long-Term Care) setting? We are seeking a motivated professional to join our team, ensuring our residents receive the best nutritional support tailored to their individual needs.

Responsibilities:

  • Provide nutritional care to residents in a Post-Acute Care (Long-Term Care) facility according to the requirements set forth by the Medicare and Medicaid Programs.
  • Develop and implement individualized nutrition plans based on residents' needs and medical conditions.
  • Monitor and evaluate residents' progress, adjusting nutrition plans as necessary.
  • Collaborate with healthcare professionals to ensure comprehensive care.
  • Educate residents and their families on nutrition and dietary needs.
  • Maintain accurate records of nutrition assessments, plans, and evaluations.

Requirements & Qualifications:

  • New York State Certification as a “Dietitian-Nutritionist” was issued by the New York State Education Department (NYSED) at the time of hiring new Dietitians (Levels I-IV).
  • Dietitians (Levels I-IV) hired before November 28, 2016, must obtain New York State Certification as a “Dietitian-Nutritionist” on or before the specified deadline.
  • NY State Office of Professionals Registered License - required.
  • Primary Source Verification - required.
  • AHA Basic Life Support (BLS) certification - required.
  • Master’s degree in Nutrition or a related field - preferred.
  • Two years of experience in a relevant field.
