The Pharmacist position will dispense drugs prescribed by physicians and other health practitioners and provide information to patients about medications and their use. The Pharmacist will also advise physicians and other health practitioners on the selection, dosage, interactions, and side effects of medications and suggest formulary substitutions as needed.
The role is responsible for safe and efficient methods for coordination of established pharmacy policy concerning drug patient care and provides clinical drug information to meet the needs of the patient and health practitioners.
Essential Position Duties
- Reviews and interprets prescription orders for appropriateness and accuracy using professional judgement.
- Ensures that drugs are appropriately labeled, stored and utilized according to physicians’ instructions.
- Dispenses prescriptions to outpatients in accordance with health center policy and in conformance with regulations and laws dealing with the class of prescription.
- Counsels patients about their medications and maintains an accurate, up-to-date patient profile.
- Provides drug information such as dosing, compatibilities and formulary substitutes to physicians and other health care practitioners.
- Participates in in-service education programs when requested.
- Arranges to obtain drugs in emergency situations.
- Dispenses wholesale prescriptions to the prescribers and prescribers’ representative, nurses, for “office use” in the medical offices at the Charter Oak Health Center.
- Responsible for supervising the pharmacy technicians and ensuring the technicians comply with all applicable pharmacy laws and regulations.
- Maintains up-to-date knowledge of the profession by participating in organization memberships; by attending meetings, lectures, in-services and seminars; by reading journals; and by complying with National Standards of Continuing Education to maintain professional competency and proficiency.
- Order and purchase pharmaceutical supplies, medical supplies, and drugs; maintaining stock, managing inventory and storing of drugs and supplies.
- Verify the use of controlled drugs by referencing the PDMP portal when necessary.
- Must attend and serve on Pharmacy and Therapeutic committees.
- All other duties assigned by management.
Compliance Responsibilities
- Complies with applicable legal requirements, standards, policies and procedures including but not limited those within the Compliance Process, Code of Conduct and Joint Commission and HIPAA.
- Participates in required orientation and training programs, as required.
- Reports concerns and suspected incidences of non-compliance in accordance with COHC Compliance reporting Process.
- Cooperates with monitoring and audit functions and investigations.
- Participates, as requested, in process improvement responsibilities.
POSITION QUALIFICATIONS
Core Competencies/Skill Sets
- Strong computer skills, including Word, Excel, and PowerPoint.
- Good staff coaching/ mentoring ability.
- Demonstrated ability to model good customer service behaviors.
- Must be thorough and have ability to remain focused in a busy environment.
- Must be able to multi-task.
- Demonstrated ability to establish and maintain effective communications among and with nursing, medical and other health disciplines.
Professional Experience/Educational Requirements
- Pharm D degree or BS in Pharmacy.
Certification/Licensure
- CT Pharmacist license, active with no restrictions. Current/ Valid CT License.
