Job Summary:
The Green House model is about building a human habitat where elders and staff are living rich and meaningful lives. At the same time, elders being served have complex nursing needs. Nurses need to assess and meet those clinical needs while supporting the quality of life of each elder. Nurses have multiple roles to fulfill to be effective in meeting the needs of elders and staff.
Nurses operate a:
- Care role model - recognizing he strengths and abilities of each elder, supporting these strengths to minimize excess disability and ensuring that care approaches are person-directed and allow the elder maximum control over his/her care and life.
- Gerontological nurse - bringing one's skills, expertise and speciailized training in assessment, care and treatment of aging people.
- Care team partner - knowing and respecting the contributions of each individual on the care team - the elders, Shahbazim and fellow CST members. As care team partners, all individuals involved in care must share information, listen to one another, share power and foster creativity by gathering multiple perspectives in any issue or situation.
- Mentor and teacher - acting as trusted guides and advisors to the Shahbazim, as well as imparting knowledge and teaching skills.
Essential Functions:
- Conduct clinical and physical assessments of elders
- Assessment of each elder for unresolved/untreated pain. Plan and implement pain management when appropriate.
- Assess, plan, and implement interventions related to: skin integrity, bowel and bladder status, including incontinence and bowel movement monitoring, mentation, ambulation status, nutrition (weights) and hydration, and the effects of medication
- Conduct MDS assessments, monitor infection control and care planning activities
Minimum Qualifications:
- Must be Licensed Practical Nurse or Registered nurse listed on the MN Board of Nursing Registry
- CPR/First Aid
- Long term care experience
