Full-time, Part-time, Per Diem positions available. Main office is in San Luis Obispo with satellite work offices in Santa Maria and Paso Robles

Overview of Position

Under the direction and supervision of the Clinical Supervisor, the LVN performs a wide range of professional duties that are related to nursing care. Assigned duties are performed within the framework of policies and standards set by management.

ESSENTIAL FUNCTIONS OF THE JOB - SUMMARY
1. Preparation of accurate and timely clinical and progress notes according to Plan of Care.
2. Assists the physician and registered nurse in performing specialized procedures as needed.
3. Preparation of equipment and materials for treatments (observing aseptic technique as required).
4. Teaches the patient/patient caregiver appropriate self‐care techniques.
5. Contacts the attending physician as needed regarding any change in patient status.
6. Clarification of new orders, including discontinued or revised orders, updates appropriate documentation, and communicates the
changes to the Case Manager/Clinical Supervisor.
7. Documents changes in wounds such as size, improvements, and deterioration according to agency policies and procedures.
8. Maintains adequate supplies and medications in patient's home.
9. Conducts a complete patient assessment during each visit to assess the total needs of the patient and update nursing care plans to
meet those needs.
10. Maintains accurate medication records.
11. Prepares patients for diagnostic and therapeutic procedures, treatments, and examinations.
12. Coordinates nursing care with the interdisciplinary team (i.e., PT, ST, OT, MSW, MD)

Core Performance Requirements

1.Completes a minimum of five patient visits in an eight‐hour workday and meeting related productivity standards established by the Agency.

2. Demonstrates effective problem solving, good judgment, and confidential handling of sensitive information.

3. Adheres to applicable State and Federal regulations governing home health/hospice policies and procedures.

4. Communicates professionally and courteously with patients/families, colleagues, and supervisors, along with no serious patient complaints reported in this past year.

5. Conducts safe practices on the job leading to 1% or fewer clinical errors.

6. Demonstrates accurate and timely documentation in accordance with established regulations.

7. Establishes and maintains successful Clinician/Patient relationships and achieving 90% or above satisfaction rates (patient/family surveys).

Qualifications and Requirements

  • Graduation from a state‐approved vocational school of nursing.
  • Currently licensed as a vocational nurse in the State of California.
  • Demonstrates knowledge of and ability to apply basic principles of nursing care.
  • Experience/education in senior care, chronically ill, restorative nursing, and/or psychiatric nursing.
  • One or more years' experience as a staff nurse desirable and Med‐Surg experience preferred.
  • Current venipuncture experience required, and state certification is desirable.
  • Current CPR card required.
