Duration: 13th weeks + Extension
Shift Timings: 8:30 AM – 5:00 PM
Weekly Hours: 30 Hrs – 40 Hrs
Pay rate: $60/hr to $65/hr
Required exp: Valid RN license in the state of New York,2 years of acute care, and recent experience in assisted living homes and current adult care home health experience.
Job Description:
The RN Adult care (Home Care) will provide comprehensive nursing care to adult patients in their homes.
Key Responsibilities:
Deliver personalized nursing care to adult patients, including assessment, planning, implementation, and evaluation of patient care plans.
Conduct regular health assessments, monitor vital signs, and manage chronic health conditions.
Administer medications, monitor for side effects, and educate patients and their families about proper medication usage.
Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to develop and implement comprehensive care plans.
Provide health education to patients and their families to promote understanding and management of health conditions.
Maintain accurate and timely documentation of patient care, progress, and outcomes.
Ensure compliance with all state and federal regulations, as well as company policies and procedures.
Qualifications:
Licensure: Current and valid RN license in the state of New York.
Experience: Minimum of two years of acute care experience in MedSurg, Maternal and Child Care, Critical Care, and Pediatrics. Must have recent experience in assisted living homes and current adult care home health experience.
Certifications: Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification preferred.
