Job Summary: The Continuous Care Nurse (LVN) is responsible for delivering comprehensive nursing care to patients who require ongoing, continuous care due to chronic illnesses, disabilities, or terminal conditions. This role involves working in various settings, including patients' homes, hospice facilities, and long-term care environments. The Continuous Care Nurse (LVN) collaborates with interdisciplinary teams to ensure the highest quality of care and support for patients and their families.

Key Responsibilities:

  1. Patient Care:

    • Provide direct patient care, including administering medications, wound care, and monitoring vital signs.
    • Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding.
    • Implement and follow individualized care plans in accordance with physician orders and nursing standards.
    • Monitor and document patient conditions, reporting any significant changes to the healthcare team promptly.

  2. Clinical Support:

    • Collaborate with registered nurses (RNs), physicians, and other healthcare professionals to ensure cohesive and comprehensive care.
    • Assist with the development and revision of patient care plans based on ongoing assessments.
    • Perform routine medical procedures such as catheterization, tracheostomy care, and blood glucose monitoring.

  3. Patient and Family Education:

    • Educate patients and their families on disease management, treatment plans, and palliative care options.
    • Provide emotional support and guidance to patients and their families, helping them understand and cope with the patient's condition.

  4. Documentation and Compliance:

    • Maintain accurate and up-to-date patient records, ensuring compliance with legal and regulatory requirements.
    • Document all nursing interventions, patient responses, and significant observations in the patient's medical record.

  5. Quality Improvement:

    • Participate in quality improvement initiatives and adhere to best practices to enhance patient outcomes.
    • Stay informed about the latest advancements in nursing care and continuous care practices.

  6. Safety and Infection Control:

    • Adhere to all safety protocols and infection control procedures to ensure a safe environment for patients and healthcare providers.
    • Use personal protective equipment (PPE) appropriately and maintain a clean and safe working environment.

Qualifications:

  • Current and valid LVN license in the state of employment.
  • Graduation from an accredited nursing program.
  • Minimum of 1 year of nursing experience, preferably in a continuous care, hospice, home health, or long-term care setting.
  • Current CPR/BLS certification.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Strong organizational skills and attention to detail.
  • Compassionate and empathetic approach to patient care.

Physical Requirements:

  • Ability to lift and move patients as required.
  • Prolonged periods of standing, walking, and physical activity.
  • Capability to perform tasks requiring manual dexterity.

Work Environment:

  • May require travel to patient homes or other care facilities.
  • Exposure to various medical conditions and environments.
  • Flexible hours, including nights, weekends, and holidays, may be required.
