Job DescriptionJob Description

Leads Certified Community Behavioral Health Clinic (CCBHC), Treatment of Individuals Experiencing Homelessness (TIEH) and Gateway Mobile Outreach Team (GMOT) program evaluation efforts, ensuring accurate and timely data entry, data reporting, and data analysis. Generates related content requirements for contractually required reports. In support of data-driven program improvement, provides recommendations based on data analysis to management and quality improvement staff.

 

DUTIES:

· Works closely with CCBHC, TIEH and GMOT Program Director to ensure complete, accurate and timely collection and reporting of NOMs assessments, including baseline, six month and discharge assessments.

· Oversee data entry for all CCBHC, TIEH and GMOT required documentation, per grant requirements.

· Provides quality control for data collection and data entry, that missing data is identified and collected, that data entry is error free and within time-frame required by grant.

· Ensures protected health information is communicated, received and protected in compliance with federal (HIPPA), state and local laws and regulations, including California law as it pertains to reporting data breaches.

· Works closely with CCBHC, TIEH and GMOT Program Director to ensure compliance rates for data collection and reporting are within targeted percentages.

· Generates statistical analysis and identifies statistical significance for data, using SPSS or comparable statistical analysis tool to measure program effectiveness and program improvement.

· Generates narrative summarizing and detailing findings for incorporation in reports and quality improvement program.

· Advances data-driven decision making program improvement by providing guidance to quality improvement staff based on data analysis.

· Works closely with CCBHC, TIEH and GMOT Program Director to ensure timely and complete submission of annual and semi-annual reports as required by grant.

· Participates in grantee meeting as required.

EDUCATION:

· Master’s degree in social sciences, public health or related field

EXPERIENCE:

· Must have at least 3 years’ experience in program evaluation

· Evaluation experience in SAMHSA grants strongly preferred

RESPONSIBILTIES

Reporting to the SAMHSA Program Director, the Evaluator is responsible for the following:

· The Evaluator will participate in meetings with the CCBHC Project Director and GPO to assure achievement of Program goals and objectives.

· The Evaluator will submit a monthly program report to CCBHC Project Director, Chief QA Officer, Chief Financial Officer and Chief Executive Officer.

· The Evaluator will prepare and submit annual reports, one each SAMHSA program on progress, barriers and outcomes.

· The Evaluator will assist the CCBHC and TIEH Program Director to complete the yearly SAMHSA continuation application to assure funding for the program.

· The Evaluator will attend the SAMHSA CCBHC discretionary program grantee’s conference if scheduled.

· Assist Program Director and Contract monitor in implementing CCBHC CQI plan by collecting, analyzing and evaluating data in support, including clinical outcome, EBP fidelity, and CCBHC specific events.

· Develop capacity to track SAMHSA required QA indicators.

· Assist in Policy and Procedure development.

· Demonstrate expertise in maintaining quality control activities including, tracking document submissions and interpreting statistical information to ensure documentation accuracy.

· Collect and analyze outcome data associated with the CCBHC and TIEH grant by monitoring staff’s completion of required Outcome Measures to ensure staff are administering and entering information into Carelogic and/ or other identified data collection system.

· Collaborate with CCBHC Program Director to develop a written protocol manual to ensure service delivery reflects the required standards of care for CCBHC and TIEH Programs.

· Participate in the agency’s accreditation/re-accreditation process.

· Support data collection for the agency.

· Attend and participate in agency’s department trainings and meetings, when required.

· Organize and prioritize multiple activities to meet all external and internal deadlines.

· Collaborate and consult effectively with the Program Directors/ Supervisors, Chief Quality Assurance Officer and other staff/programs to ensure Quality Assurance procedures are followed.

· Generate Carelogic reports as need to assist staff with compliance.

· Other duties not specified in the position description as requested by the CCBHC and TIEH Program Director, Chief Operating Officer or Chief Executive Officer.

· Assist GMOT Director in the evaluation of data collected and extraction with key performance indicators

· Assist GMOT Director in collecting and reporting on established metrics by providing qualitative data and narrative to the Gateway Cities Council of Government on a monthly basis
