What will you do at CHOICE?

  • The HARP Intensive Case Manager is responsible for managing the HARP assessment process from start to finish for clients enrolled in the Health Home program.
  • Educating clients on HARP and Home and Community Based Serves (HCBS).
  • Completing the HARP assessment with eligible clients, completing the Plan of Care, and working collaboratively with the interdisciplinary care team, specifically Managed Care Organizations and HCBS Providers to coordinate services.

Each client may bring something different on any given day that may make you step outside of your traditional role. Care Coordinators should be coming in with an open mind to what the day may bring.

Anything else you can bring to the table is a plus

Qualifications for the role:

The Care Manager must have a bachelor’s degree in a Human Services field or at least 5 years working with clients in the Mental Health Field  In addition to BA, the Care Manager will have at least two years of experience working with adults with serious mental illness. A Master’s Degree in the Human Services field will count as one year of the two years of required experience.  Tenacity and passion for this work with the ability to balance objectivity with empathy.  Computer literacy is required.  Knowledge of behavioral diagnoses and symptomatology  Working familiarity with community resources, including entitlement programs, medical, financial, and legal services, housing and emergency food programs, and behavioral health treatment and rehabilitation services. Must be able to effectively engage clients.  A valid NYS Driver’s License is required and a driving record that will permit the use of an Agency vehicle to transport clients to appointments or other activities as needed.
