Job Title: Certified Nurse Assistant (CNA) - Home Health

Job Description: As a Certified Nurse Assistant (CNA) working in home health, your primary role is to provide compassionate and personalized care to individuals in the comfort of their own homes. You will assist clients with activities of daily living, monitor their health status, and ensure their safety and well-being while promoting independence and dignity.

Key Responsibilities:

  1. Personal Care Assistance: Assist clients with bathing, dressing, grooming, toileting, and other activities of daily living as needed.
  2. Mobility Support: Help clients with mobility issues by providing assistance with transferring, walking, and using mobility aids.
  3. Vital Signs Monitoring: Measure and record vital signs such as temperature, pulse, respiratory rate, and blood pressure, reporting any abnormalities to the supervising nurse or healthcare provider.
  4. Medication Management: Assist clients with medication reminders and administration under the direction of a nurse or healthcare professional.
  5. Wound Care: Provide basic wound care, including cleaning, dressing changes, and monitoring for signs of infection.
  6. Nutrition Support: Assist with meal preparation, feeding, and ensuring clients adhere to any dietary restrictions or special diets prescribed.
  7. Emotional Support: Offer companionship, reassurance, and emotional support to clients, engaging in conversation and providing social interaction.
  8. Household Tasks: Perform light housekeeping duties such as laundry, cleaning, and tidying up the client's living space to maintain a safe and sanitary environment.
  9. Documentation: Maintain accurate and up-to-date records of care provided, including daily activities, vital signs, medication administration, and any changes in the client's condition.
  10. Collaboration: Work closely with other members of the healthcare team, including nurses, therapists, and social workers, to ensure coordinated and comprehensive care for clients.
  11. Client Advocacy: Advocate for the needs and preferences of clients, ensuring they receive person-centered care that respects their dignity and autonomy.

Qualifications:

  1. Certification as a Nurse Assistant (CNA) in the state of practice.
  2. High School Diploma or equivalent.
  3. Strong interpersonal and communication skills.
  4. Compassion, patience, and empathy.
  5. Ability to follow instructions and work independently.
  6. Physical stamina and ability to lift and move clients with proper techniques.
  7. Valid driver's license and reliable transportation may be required for certain assignments.
