Job DescriptionJob Description

Life Coaching 4 Kids Center Counseling Department is seeking an enthusiastic case manager to work with children receiving behavioral-health services as they progress on the path to a more self-sufficient lifestyle and potential family reunification. The Case Manager also provides guidance and ongoing support to the families/caregivers of the youth receiving services.

Duties include:

  • Provides case management services to children enrolled in the behavioral-health system.

  • Provides weekly contact with assigned clients, which includes home/community visits, attending medication checks, etc.

  • Assists youth in navigating discharge from hospitals, in finding intensive services as clinically indicated, and working to decrease behaviors to improve stability in the home & community.

  • Coordinates services and collaborates with other involved agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.

  • Coordinates and facilitates Child & Family Team (CFT) Meetings.

  • Completes referrals, prior authorizations, and other paperwork required by the health plans to enroll clients in services.

  • Collaborates with clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork is completed accurately and in a timely manner.

Qualifications:

-Meets one of the following education/experience requirements:

  • High School Diploma/GED required and 4 years of Behavioral Health experience.

  • A.A. Degree and two years of Behavioral Health experience

  • Bachelor’s Degree in Social Work, Psychology, or related field.
